Produces from a word processor a document that is almost identical to the official BSA Training Attendance Report; you just have to type in the list of attendees.
Originally formatted for MS Word for Windows 2.0, but I have imported it into WordPerfect and higher levels of Word without problems.

Librarian's Note: This 'form' may need to be adjusted for different word processors, including changes to font and size. Every word processor has its own intricacies, and just about every user has imposed his own preferences on those intricacies.
In Claris Works it comes up in the Chicago font for me. In Microsoft Word 5.1A it shows as only a single line blank table.
PS
In MS Word for Windows this came up as a template with one line for attendees, which can be copied as needed to add other people. Since it IS a template for MS Word type word processors, it is difficult to imagine how one could modify, or even display the outline of the form in any other word processor. If this one doesn't work with YOUR word processor, please contact the author and see if he has made a template for your program, .
